Cancer and Chronic Disease Data Management System Enhancement and Maintenance

The State of Delaware, Department of Health and Social Services (DHSS), Division of Public Health (DPH) is seeking a contractor to provide professional services for the enhancement and maintenance of the Cancer and Chronic Disease Data Management System (CCDDMS), formerly known as the Screening for Life (SFL) system. This system is crucial for supporting DPH's statewide strategies to mitigate the impact of chronic disease risk factors associated with cancer, hypertension, diabetes, asthma, and other chronic conditions. The goal is to integrate the existing SFL system with the Smart Sheets data collection tool to streamline data collection, improve federal and state reporting, and establish a robust centralized hub for decision-making. The scope of work includes maintaining and enhancing the CCDDMS, supporting data conversion, generating reports, developing queries, creating data visualization dashboards, providing ongoing support for program applications, hosting databases securely, integrating patient navigation processes, developing data dashboards for specific programs (CDSMP, HHABPSM, EMDF), developing a pharmacy claims reimbursement process, facilitating electronic claims submission, and establishing and maintaining a Smartsheet license for various tracking and data collection purposes. The system should also offer a Spanish language option on the login screen and ensure accurate translation of all components.
