Chronic Disease Community Support Programs

The City of Kansas City, Missouri, through its Health Department, is seeking proposals from qualified community-based organizations to implement initiatives aimed at reducing chronic disease risks through behavior change support. Three contracts of up to $40,000 each will be awarded to organizations serving residents within the North, Central, and South Health Prosperity Zones, with a preference for those targeting residents with the lowest health opportunities.
