The State of Delaware, Department of Health and Social Services, Division of Substance Abuse and Mental Health (DSAMH), is seeking proposals from qualified organizations to provide Community-Based Residential Alternative services in a Group Home setting for individuals enrolled in the Promoting Optimal Mental Health for Individuals through Supports and Empowerment (PROMISE) program. This RFP is specifically for services based in Sussex County, Delaware. The selected providers will operate group homes licensed by the State of Delaware's Division of Health Care Quality and obtain/maintain Medicaid certification, offering varying degrees of support for individuals with severe and persistent mental illness (SPMI) and significant substance use disorders (SUDs) requiring 24/7 care. The group homes will serve adults aged 18 and older who are residents of Delaware and meet specific clinical acuity criteria for Tiers 3, 4, and 5. Services will be provided in a home-like setting for five to ten individuals per home, focusing on person-centered planning, recovery-oriented services, and integration into the community. The goal is to provide a range of support levels within the group home setting to help beneficiaries live in the most integrated setting possible, with a focus on developing skills for independent living and community participation.
