The VA Northern California Health Care System is seeking proposals for an Indefinite Delivery Indefinite Quantity (IDIQ) contract to provide community based outpatient clinic (CBOC) services in Yreka, CA. The contract will cover a period of performance from September 1, 2026, to August 31, 2036. The services will be provided at a CBOC facility located within the incorporated city limits of Yreka, California, serving beneficiaries of the VA Northern California Health Care System. The contractor will be responsible for providing comprehensive primary care services, including managing preventive and chronic care, coordinating care within the VA PACT model, and ensuring patient access through various modalities such as face-to-face visits, virtual care, and telephone access. The contract also includes requirements for women's health services, mental health services, and ancillary support services like radiology and laboratory testing. Staffing requirements include a Physician Director, Primary Care Providers (MD/DO, NP/PA), Registered Nurse Care Managers, Clinical Associates, Administrative Support, and potentially specialized staff such as Clinical Pharmacist Practitioners, Licensed Social Workers, and Registered Dietitians. The contractor must adhere to all VA performance and quality criteria, including those related to access, patient safety, and satisfaction. This includes compliance with VA policies, HIPAA, and information security requirements.
