The State of Delaware, Division of Public Health, is seeking proposals for professional services to continue and expand the Parents as Teachers (PAT) program. This initiative aims to build the capacity of the PAT program by targeting high-risk communities identified in the Maternal, Infant and Early Childhood Home Visiting (MIECHV) Needs Assessment. The project is funded by the MIECHV grant from the U.S. Health Resources and Services Administration (HRSA) and administered by the Division of Public Health. The scope of services includes implementing and sustaining a continuum of home visiting services statewide, with a focus on fidelity to the PAT model and delivering services in at-risk community zones. The program aims to improve maternal, infant, and early childhood outcomes through targeted evidence-based home visiting services, and to monitor system changes for long-term sustainability. Key performance measures include preterm birth, breastfeeding rates, depression screening, well-child visits, postpartum care, tobacco cessation referrals, safe sleep practices, child injury rates, child maltreatment, parent-child interaction, early language and literacy activities, developmental screening, behavioral concerns, intimate partner violence screening, primary caregiver education, continuity of insurance coverage, and completed referrals for depression and developmental concerns.
