Small Works Roster Application for Public Works Projects

Quillayute Valley School District Number 402 is soliciting contractors to be placed on their Small Works Roster for public works projects. Projects are anticipated to cost less than $350,000. Contractors must agree to provide prevailing wages for their employees and complete all necessary intents and affidavits required by the State of Washington Department of Labor and Industries. These forms must be completed for each job and received by the district office before invoices will be processed for payment. Placement on the Small Works Roster is contingent upon receipt of a properly completed application form, a current certificate of insurance, and an IRS Form W-9. The District may negotiate directly with individual contractors for projects estimated under $150,000, based on qualifications, availability, and the District's Business Utilization Plan goals for small, minority, women, and veteran-owned businesses.
