Blanket Purchase Agreement for Triage D-Dimer Test Kit Supplies

This solicitation is for miscellaneous Triage D-dimer test kit supplies for the Indian Health Center, Four Corners Regional Health Center, located at JCT US Highway 160 & Navajo Route 35, Red Mesa, AZ 86514. The contract will be a Blanket Purchase Agreement with a base year and four option years. The opportunity is set aside 100% for Indian-Owned Small Business Economic Enterprises (ISBEE). Offerors must be registered in the System for Award Management (SAM) and meet the definition of an "Indian Economic Enterprise" throughout the offer, award, and performance periods. Key items include Triage Cardiac Panel, Troponin I, Troponin I and CK-MB, Total Controls, Calibration Verification, BNP, D-dimer Test, and TOX 9 Test Panel supplies, as detailed in Attachment A - Triage D-Dimer Test Kits Supply List.
