Rural Health Transformation Program Support Services

The State of Delaware's Division of Public Health is seeking qualified vendors to provide grant program oversight and/or evaluation services for the Rural Health Transformation Program (RHTP). This program aims to expand access, strengthen the workforce, modernize technology, and improve population health in rural Kent and Sussex Counties. The selected vendor will be responsible for supporting the coordination, monitoring, and execution of implementation activities necessary to advance initiative milestones and meet federal grant requirements. Services include program coordination across initiatives, budget adherence, compliance monitoring, and facilitating communication among subrecipients. Additionally, the vendor will develop and implement a comprehensive evaluation framework to measure process and outcome metrics, conduct ongoing monitoring of subrecipient performance, and provide actionable data insights to support strategic decision-making. The vendor will also assist with data visualization and tracking tools using the AchieveIt platform.
