Licensed Optometrist Services - Amended

This is a total Small Business set-aside for licensed optometrist services at the Kayenta Health Center. The contractor will provide comprehensive eye care, diagnose and manage visual and ocular pathologies, prescribe corrective lenses, administer vision training, and use pharmacological means to treat visual disorders. The contractor will also provide emergency eye care services, utilize laboratory and radiology services, coordinate medical eye care with other physicians, and follow KSU optometry policies and procedures. Key personnel submissions must be qualified, applicable, and available.
