Oral and Maxillofacial Surgery Services

University Medical Center of Southern Nevada (UMC) is soliciting information from various providers, including physician groups and individuals, who can offer oral and maxillofacial surgery services. This Request for Information (RFI) aims to identify potential providers for on-call and clinical services. Respondents are required to review and complete the provided Professional Services Agreement (Exhibit A) and other accompanying exhibits, including Representations and Certifications (Exhibit D) and a Vendor/Supplier Self-Identification Form (Exhibit E).
